VNA brings home-care expertise to UVM Health Network

The Visiting Nurse Association of Chittenden & Grand Isle Counties becomes the seventh UVM Health Network affiliate

In October, the Visiting Nurse Association of Chittenden and Grand Isle Counties (VNA) Board of Directors voted unanimously to formally affiliate with the UVM Health Network. The agreement took effect January 1, 2018.

Following the signing of a Letter of Intent in May, the two organizations explored the benefits and impact of the VNA’s becoming the first affiliate dedicated to providing post-acute, community-based care.  It is Vermont’s oldest and largest home health and hospice agency. The VNA of Chittenden and  Grand Isle Counties will remain a nonprofit organization with its own Board of Directors, responsible for its own balance sheet and fundraising efforts.

The VNA of Chittenden and Grand Isle Counties has played an essential role in the care of our communities. This affiliation acknowledges the ongoing changes in health care delivery and the importance of greater integration and collaboration among local health care providers. It is built on the premise that caring for people where they are, at all stages of their lives, is essential to the overall health of our community. By integrating acute, post-acute and long-term care, chronic disease management and end-of-life care, we will be better able to serve our patients.
